Vision Insurance

The City of Fontana offers optional coverage through Anthem Blue View Vision. The Policy provides full coverage for Covered Services, less a $10 copay for the examination, when you go to a Participating Provider of Anthem Blue Vision. While benefits and savings are typically greater from in-network providers, members are free to visit an out-of-network provider. You have the option of receiving an eye exam from one provider and filling your prescription at another location.

Additional Life Insurance and Accidental Death & Dismemberment (AD&D)

  • Additional Life insurance can be purchased in multiples of $10,000 to a maximum of $500,000 or 5 times annual salary, whichever is less
  • The cost for additional life insurance is based on an employee's age
  • A Guaranteed Issue coverage amount of $110,000 (employees only) may be purchased during the employee’s initial benefits enrollment or during open enrollment
    • An EOI form is required when purchasing Additional Life insurance above the guaranteed issue coverage amount
    • The EOI is subject to approval by the underwriting company and may require a medical exam

Optional Spouse Life/AD&D

  • An employee may apply for Optional Spouse Life insurance in multiples of $5,000 to a maximum of $300,000
  • The cost for Optional Spouse insurance is based on the spouse's age
  • A Guaranteed Issue coverage amount of $25,000 may be purchased during the employee’s initial benefits enrollment or during open enrollment
    • An EOI form is required when purchasing additional life insurance above the guaranteed issue coverage amount
    • The EOI is subject to approval by the underwriting company and may require a medical exam

Optional Dependent (Children) Coverage

Optional Life insurance may be purchased for dependent child(ren) as long as the employee is enrolled in additional life insurance coverage. Children are eligible from 1 year to 26 years of age.

  • $0.95 per month for $5,000 death benefit (for one or more dependents)
  • $1.90 per month for $10,000 death benefit (for one or more dependents)

Age Calculation

  • Age for life insurance is calculated once a year at open enrollment and is based on the age which the employee will turn during the coverage year despite the month at which this occurs. For example, if an employee will turn 40 in October during the following coverage year, the employee’s monthly rate will be based off of age 40 for all 12 months. Spouse ages are subject to the same annual calculation.
